RPM and CCM: How the Two Programs Work Together

Two programs, two jobs
Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) are often mentioned together, but they solve different problems. RPM collects physiologic data from patients at home. CCM provides ongoing care coordination between visits for patients living with multiple chronic conditions.
What RPM does
RPM uses connected devices, such as blood pressure monitors, glucometers, pulse oximeters, and weight scales, to send readings to the care team. Reviewed regularly, those readings show trends and changes between visits and help teams respond when attention is needed.
What CCM does
CCM supports patients with two or more chronic conditions through a personalized care plan, regular check-ins, medication management, and coordination with specialists and other members of the care team. Its focus is the whole patient, not a single measurement.
Why they work better together
RPM provides the data. CCM keeps the care plan moving. When a blood pressure trend rises, a CCM check-in is a natural moment to review medications, adherence, and next steps with the patient. When a CCM conversation raises a concern, RPM readings add context.
Who may qualify
Eligibility depends on payer rules and clinical judgment. In general, RPM is used for patients whose condition benefits from regular physiologic monitoring, and CCM is used for patients with two or more chronic conditions expected to last at least 12 months. Your providers make the final call, and current Medicare requirements should be confirmed with your billing team.
